5M complies with the NDIS Code of Conduct and therefore has many policies and procedures which guide how we work. These policies are available on request.
The National Standards for Disability Services are available in Easy-read English. The standards identify your rights as a service user.
These are determined by the participant when the first plan is developed. They may be amended at any time by contacting the NDIA.
This is the online management system linked to your My Gov account. Invoices are sent directly to the NDIA for payment
This is an agreement between the service provider and the participant and lists the responsibilities of each party
This is the money side of the business, it details your NDIS plan budget, how much service providers will get paid and how and when they will provide service.
If your plan is agency managed, the service provider will book or reserve the full amount to their organization. This is not a payment yet.
Once service has been provided, the service provider will raise an invoice and claim the money from the NDIS.
