Accommodation/Tenancy

We provide short-term respite Accommodation for when you just need a break. We understand and we are here to cater to your needs for whenever you just need to get away. Accommodation / Tenancy also aims to assist you in applying for suitable rental accommodation or undertake tenancy obligations in line with the participant’s tenancy agreement and needs. We ensure that our NDIS participants obtain or retain appropriate and safe accommodation. 5M can provide one on one support to assist participants to achieve their individual Accommodation and Tenancy goals such as assisting participants to overcome barriers that impact a current tenancy or supporting participants to explore and obtain alternative housing. Our Accommodation and Tenancy services includes assessment, individual case planning, practical one on one advice, liaising with service providers and capacity building assistance to support people to achieve their individual goals.

Accommodation/Tenancy

Assistance Daily Personal Activities

5M is committed to giving back to the community and is proud of having experienced support workers who are determined to provide only the best services to all participants. Our Assist Personal Activities include providing support that will make your life comfortable and easy. Our team of professionals are well trained and will support you in daily living. The program aims to assist with, and/or supervise, personal tasks of daily life to develop the skills of the participant to live as autonomously as possible. This includes but not limited to:

• Showering
• Personal hygiene & grooming
• Dressing
• Going to the toilet
• Getting in and out of bed
• Moving around the house
• Meal preparation
• Help with eating

Assist-Life Stage Transition

Assistance with life stage transition service includes short and long terms support that aims at strengthening and supporting the participant’s abilities and choices. Transitions such as moving out of one’s home, joining the university, or getting a job are part of everyone’s life. It is never an easy task both emotionally and practically, and it becomes a little more difficult for people with disabilities. Change can be hard and with it comes new environments and settings. Under this NDIS support category, 5M offers a wide range of supports to assist life stage transitions. The idea is to make this transition phase as smooth as possible without disrupting the comfort of the participant. We work with the participant to enable a friction-free move and encourage the participant to settle in a new setting and create a comfortable attitude. Some our services for this purpose include:

• Support connection and coordination
• Capacity building skills
• Development of independent self-care and living skills
• Crisis and conflict resolution
• Daily planning and Budgeting
• Establishing supportive networks in the participant’s communal setup
• Linking our support to other supportive community services and programs
• Assistance with accommodation, tenancy procedures

• Mentoring and peer support
• Individual skill development

Assist Travel/Transport

5M provides travel and transport services to people who need help getting around. Assist / Travel Transport is designed for participants who cannot use public transport due to their disability. Our transport assistance professionals are highly qualified with valid driver’s license and police background checks. This service includes travel arrangements made with personal or public transport systems for the participant to attend their school, educational facility, employment, appointments or community events. We provide safe and convenient transport services to help you in your journey.. We make your mobility easier and much safer for you.

Community Nursing Care

At 5M Community Services, we love helping our participants who are in need of nursing services. These services are designed to not only empower you to do things yourself but also give you self-confidence and the independence you need. That is why 5M Community services will provide full nursing support in the assessment and training of nurses to improve daily living. We will allow a nurse to train you or a support worker or carer for supports that are related to your disability such as wound care or even medication prompting. Our highly skilled registered nurses promote a healthy lifestyle and provide nursing disability care services both when a participant is ill and to prevent illnesses. Our Community Nursing Care staff are caring professionals who are highly skilled, culturally sensitive and ready to help. These clinical and personal nursing services can be provided in the comfort of your own home. Community nursing is a great form of care because it aims to support your independence and your lifestyle in a safe and familiar environment.

Community Nursing Care

At 5M Community Services, we love helping our participants who are in need of nursing services. Daily task/shared living

This service incorporates assistance with and/or supervising tasks of daily life in a shared living environment, which is either temporary or ongoing, with a focus on developing the skills of each individual to live as autonomously as possible. This support service also includes short-term accommodation and assistance (e.g. respite care). We understand that living independently can sometimes feel impossible, but with our support we can make it possible. Our expert staff will support you in every way, be it teaching you to live with others while ensuring you can fulfill your responsibilities around the home, assisting you with your move into a new house or even assisting you with Daily Tasks like getting ready, preparing meals and enjoying your life without having to depend on others.

Development-life-skills

With the Development-Life Skills program, we encourage participants to develop individual life skills to help them gain independence, knowledge and confidence. This includes special personality development life skills, behavioural life skills development, daily living support services and more. We provide training including public transport training and support, developing skills for community, social and recreational participation. 5M will engage with you to understand your goals and work with you and your family/carers to put in place supports to develop everyday skills with a focus on building confidence and independence. We can assist you in acquiring new life skills that will have positive impacts on your entire life.

Development-life-skills

With the Development-Life Skills program, we encourage participants to develop individual life skills to help them gain independence, knowledge and confidence. This includes special personality development life skills, behavioural life skills development, daily living support services and more. We provide training including public transport training and support, developing skills for community, social and recreational participation. 5M will engage with you to understand your goals and work with you and your family/carers to put in place supports to develop everyday skills with a focus on building confidence and independence. We can assist you in acquiring new life skills that will have positive impacts on your entire life.

Group/Centre Activities

5M Community Services offers an inclusive environment and the opportunity for learning, friendship, growth and most importantly, fun. We encourage participants to engage in activities independently and/or participate in workshops/group activities. Bonding and social connections are vital to individuals, we understand life is more enjoyed together. Our Group / Centre Activities assist clients to participate in the group-based community, social and recreational activities, such as movies, beaches, zoos, arts and crafts and outdoor activities such as parks, exploring the greater Penrith and Sydney areas. Enjoy a wide range of social experiences, activities and develop your skills with us.

Assistance with Household Tasks

We are certified NDIS providers who can assist you with Household Tasks of daily life. We will sort out any essential Household tasks that you are not able to perform independently. Essential cleaning is offered across all areas that are extensively used by our client within their home. As a rule, if you can’t perform a task – like vacuuming, cleaning, home and garden maintenance, mowing the lawn, cleaning, laundry – because of your disability, then 5M support workers can have that sorted for you. From general to deep cleaning, from edging and mowing to hedging and weed control, these are just examples of what 5M provides. We are the best NDIS Service provider who can assist you in household tasks that can become challenging for you. Whatever is the household task, our group of fully trained staff guarantees that we will do it for you.

Innovative Community Participation

It also develops independence and confidence around other members of the society. We can assist you in connecting with the community by supporting you and participating in the activities you enjoy while also learning new skills.

Participate Community

The Participate Community program enables social and community participation for people with disabilities.
